At Archer's Total Home Healthcare, we understand the importance of protecting your health information. We are required by law to maintain the privacy of your Protected Health Information ("PHI"), provide you with this Notice of our legal duties and privacy practices, and comply with the terms of this Notice.

Your Protected Health Information

PHI includes information that identifies you and relates to your health condition, healthcare services, or payment for healthcare services.

How We May Use and Disclose Your Information

Treatment

We may use and disclose your PHI to physicians, hospitals, healthcare providers, and others involved in your care to coordinate treatment and provide home medical equipment and related services.

Payment

We may use and disclose your PHI to bill and collect payment from Medicare, Medicaid, commercial insurance companies, managed care organizations, or other responsible parties.

Healthcare Operations

We may use your PHI to support business activities such as quality improvement, compliance reviews, staff training, auditing, accreditation, licensing, and customer service.

Individuals Involved in Your Care

Unless you object, we may share relevant information with family members, caregivers, or others involved in your care or payment for your care.

Required by Law

We may disclose PHI when required by federal, state, or local law.

Public Health and Safety

We may disclose PHI to public health agencies, regulatory authorities, or other authorized entities for public health activities, disease prevention, reporting, and health oversight activities.

Law Enforcement and Legal Proceedings

We may disclose PHI in response to court orders, subpoenas, warrants, or other lawful processes.

Workers' Compensation

We may disclose PHI as authorized by workers' compensation laws and similar programs.

Uses Requiring Your Authorization

We will obtain your written authorization before using or disclosing your PHI for purposes not otherwise permitted by law, including most marketing uses and the sale of PHI.

You may revoke an authorization at any time in writing, except to the extent we have already acted upon it.

Inspect and Obtain CopiesReview and obtain copies of your PHI as permitted by law.
Request AmendmentsRequest corrections to your PHI if you believe it is inaccurate or incomplete.
Receive an Accounting of DisclosuresRequest a list of certain disclosures we have made of your PHI.
Receive a Paper CopyRequest a paper copy of this Notice at any time.

Our Responsibilities

We are required to:

  • Maintain the privacy and security of your PHI.
  • Provide this Notice of Privacy Practices.
  • Follow the terms of the current Notice.
  • Notify affected individuals of certain breaches involving unsecured PHI as required by law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
