Respect and Dignity
- Be treated with courtesy, respect, consideration, and dignity.
- Receive services without discrimination based on race, color, national origin, religion, age, sex, disability, sexual orientation, gender identity, or source of payment.
Information and Participation
- Receive information regarding products, services, policies, procedures, and charges.
- Participate in decisions regarding your care and equipment needs.
- Be informed of any financial obligations, including co-payments, deductibles, and non-covered items.
- Receive information regarding Medicare, Medicaid, private insurance, and other payer requirements affecting your equipment or services.
Privacy and Confidentiality
- Have your personal and health information treated confidentially in accordance with applicable laws, including HIPAA.
- Access and request amendments to your health information as permitted by law.
Safety and Quality
- Receive equipment that meets applicable safety standards.
- Receive proper instruction regarding the safe use, maintenance, and operation of equipment.
- Receive timely delivery and service consistent with physician orders and payer requirements.
- Be informed of any changes affecting services or equipment.
Complaints and Grievances
- Voice complaints, concerns, or grievances without fear of retaliation.
- Receive prompt investigation and resolution of complaints.
- Contact Medicare or other regulatory agencies regarding concerns about services received.
Freedom of Choice
- Select among qualified providers when permitted by your insurance plan or payer.
- Refuse equipment or services to the extent permitted by law.
Patient Responsibilities
As a patient, you are responsible for:
Providing Accurate Information
- Providing complete and accurate information regarding your medical condition, physician orders, insurance coverage, and contact information.
- Notifying us promptly of any changes in your health status, address, telephone number, physician, or insurance coverage.
Equipment Care and Use
- Following instructions regarding the safe and proper use of equipment.
- Maintaining equipment in a clean and safe condition.
- Notifying Archer's Total Home Healthcare of equipment malfunctions, damage, loss, or theft.
Participation in Care
- Following treatment recommendations and physician instructions related to equipment use.
- Asking questions when instructions or information are unclear.
Financial Responsibilities
- Paying applicable deductibles, co-payments, coinsurance amounts, and charges for non-covered items.
- Providing accurate insurance information and cooperating with claims processing requirements.
Respectful Conduct
- Treating staff members with respect and courtesy.
- Maintaining a safe environment for staff making deliveries or service visits.
